HCFA 1500 Form filler for Lion

Hello Mac Geeks,
My wife uses a program to fill out these forms for insuance companies to get reimbursed. she is a psycotherapist. The one she uses runs in emulation mode on snw leopard and I haven’t found a Mac program that will do this function for Lion. I would like to upgrade her MacBook pro to Lion but she needs a program to fill out and print these insurance forms. She enters the info in the fields and the program prints on pre printed forms. She has to use the forms because they are in red and the insurance company scanners need the red borders etc. Does nayone know of a program that will do this in Lion?

Thanks for all the possibilities! The one from UB-40 is the one she uses now. I’ve written to them to see if they have a version that works with Lion but no response so far. Thanks again for all of your help. We will check out the others and get back to you as to which one fit the bill:)
