Spoilers for American Horror Story Season 13 Episode 4 follow. Ben is not merely a nurse trapped in Constance’s strange household. In “Is That All There Is?”, Mr. Nas makes him drink from the same dark chalice that Nas has been using, and Dr. Phibes later refers to Nas letting his son drink from it. The episode strongly points to Ben as that son, although it does not give him a complete explanation of his own origins.

That possibility changes the meaning of Ben’s presence at the house. He has appeared to be an outsider caring for a patient. Now he may be part of the very plan that is restoring Nas. The premiere’s repeated attention to the number 13 becomes more than atmosphere as the ritual below the building supplies Nas with new strength.

Ben’s encounter with Nas may not be only a dream

Ben wanders through the apartment while drugged and finds Nas drinking from a chalice. Nas then feeds him the same substance. Ben’s altered state makes the sequence difficult to trust at first: the next morning, he wakes with Constance and Phibes beside him. Yet their conversation gives the apparent dream a concrete connection to the ceremony downstairs.

Phibes says Nas allowed his son to drink. The line makes Ben’s experience seem less like a random nightmare and more like a deliberate part of the restoration. It is still worth separating the implication from a fully confirmed family history. The episode suggests a father-son relationship, but it has not explained how Ben came to be raised outside this household or what he knows about it.

The missing thirteenth sacrifice completes the ceremony

At Club Hell, Elsa Mars performs while women connected to the season’s infernal gathering are presented as offerings. The group plans for 13 sacrifices but comes up one short. Wilhemina Venable throws herself into the pit, completing the count and setting off the fiery response that the others wanted.

Constance and Phibes later inspect Nas and find that the ceremony has pushed his recovery forward. Their discussion makes clear that the sacrifices serve a larger plan, rather than providing a spectacle unrelated to Ben. If Ben is Nas’ son, his apparent role in the chalice scene could make him crucial to what happens when Nas reaches full strength.

Constance’s bargain gives Ben another reason to be wary

Constance explains that she agreed to help Nas regain power in exchange for another chance with her children, Tate and Addie. Her wish is personal, but the cost reaches well beyond her family. She has helped assemble a ritual that harms others, while Ben has been kept close to Nas without understanding the arrangement around him.

The ending therefore answers one question and opens a more troubling one. Ben appears connected to Nas by more than his job, but whether he can refuse a role in Nas’ return remains unresolved in Episode 4.
