One Apple Watch Health Announcement, Four Different Age Limits and Two Device Cutoffs

Apple’s September 9 Apple Watch Series 12 and Ultra 4 health materials set four separate age floors, 13, 18, 22 and 40, one for each of four different features. The same materials list ECG eligibility on Apple Watch models that cannot run watchOS 27 itself.

None of these limits is buried or disputed. Each is stated plainly in Apple’s own wording, just not gathered in one place until now.

The device floor watchOS 27 itself sets

That is the baseline for the operating system update itself, published alongside the Series 12 and Ultra 4 announcements. Several of the individual health features inside that same release reach past this floor in one direction or the other.

ECG: a wider device list, and a 22-year floor

Apple’s own wording for the ECG app: it is ‘available on Apple Watch Series 4 and later (excluding Apple Watch SE models) and all Apple Watch Ultra models and can generate an ECG similar to a single-lead electrocardiogram. Intended for use by people 22 years old or older.’

Series 4 launched years before Series 9, and ‘all Apple Watch Ultra models’ includes the original Ultra, a generation earlier than the Ultra 2 floor watchOS 27 itself requires. Apple’s ECG eligibility statement is wider than its own new operating system’s device list, because ECG has existed on older software for longer than watchOS 27 has existed at all.

Sleep apnea: an 18-year floor, and only for the undiagnosed

Apple’s wording here matches the watchOS 27 device floor more closely: sleep apnea notifications are ‘available on Apple Watch Series 9 or later, Apple Watch Ultra 2 or later, and Apple Watch SE 3.’ The feature is ‘intended to detect signs of moderate to severe sleep apnea for people 18 years old or older without a diagnosis of sleep apnea,’ a lower age floor than ECG’s, and one that excludes anyone already diagnosed.

Hypertension notifications and perimenopause: 22 and 40

Hypertension notifications carry the same 22-year floor as ECG, and additionally exclude users previously diagnosed with hypertension and pregnant individuals, per Apple’s materials. Perimenopause notifications in Cycle Tracking sit at a different floor entirely: Apple states that users 40 and over get notifications when logged cycle patterns suggest perimenopause, along with separate postmenopausal bleeding notifications, not available in all regions.

Two disclaimers that are not age limits

Apple attaches two further statements that qualify use without setting an age. The Vitals app carries Apple’s own line that it is ‘for wellness purposes only and not for medical use.’ Cycle Tracking carries its own separate statement: it ‘should not be used as a form of birth control.’ Neither is an age gate, but both sit in the same release as the four numeric floors above, and both are worth reading alongside them. What Apple has not said

Apple has not published a single combined eligibility table covering age, diagnosis status, region and device generation together for these five features. It also has not stated why ECG’s device list reaches back to Series 4 and the original Ultra while sleep apnea and hypertension notifications do not, beyond the fact that ECG has been available on older software for longer. Readers checking whether a specific watch and age combination qualifies for a specific feature should check Apple’s own feature pages directly rather than assume one feature’s rule applies to another.
