According to CNN, an analyst used a chatbot to assess information and then used AI again to package the findings into a report. Human officials remained responsible for decisions and eventually identified the error.

The Pentagon has not publicly released a detailed account of the incident. Until it does, the scope, the model involved and the precise decision chain remain based on CNN’s reporting rather than official findings.
