Apple Watch Ultra 4 costs $799 and Apple Watch Series 12 costs $399, and Apple gives both the same Health Sensing System, the same S11 chip and the same health claims worded the same way. The extra $400 buys battery life, GPS, satellite, water rating and brightness.
It does not buy extra health sensing. Heart rate every five seconds, HRV as often as every five minutes, Recovery HRV against a personal baseline, the Readiness score and the sleep score are published for both, and the accuracy claim behind them is a single study cited in both press releases.

The health claims that are identical
Both press releases carry the same sentence about accuracy: a scientifically rigorous study on a diverse population of over 1,000 participants compared the new heart rate sensing accuracy on Apple Watch against leading wearables, resulting in Apple Watch showing the highest accuracy across devices. Footnote 1 of the Ultra 4 release dates that work to July and August 2026.

Neither release names a journal, an institution or the devices Apple compared against, and neither publishes a methodology or an accuracy figure. The claim is the same claim on both watches, and it is equally unverified on both.
The sensor lists match too. Both pages name an electrical heart sensor, an always-on optical heart sensor, a blood oxygen sensor and a temperature sensor, plus compass, always-on altimeter, high-g accelerometer, high dynamic range gyroscope, ambient light sensor and a water temperature sensor.
The only wording differences
Two. The Series 12 page measures its frequency claims against Series 11 while the Ultra 4 page measures them against Ultra 3, and the Ultra 4 page phrases the HRV figure as often as every five minutes where the Series 12 page leads with the every-five-seconds heart rate line. The rates themselves are the same.
The depth gauge is the one sensor scoped differently. Series 12 lists a depth gauge to 6 meters. Ultra 4 is rated to 100 meters under ISO 22810:2010 and certified to EN13319 for recreational scuba to 40 meters, with depth accuracy Apple states as plus or minus 1 meter.

Wrist fit differs as well. Series 12 bands cover 130 to 200 mm at 42mm and 140 to 245 mm at 46mm, while Ultra 4 covers 130 to 210 mm. The 49mm case is the constraint on Ultra 4, not the sensing.
How Apple labels all of it
Apple presents this as wellness, not medicine. The Vitals app is described as for wellness purposes only and not for medical use, Cycle Tracking is not to be used as a form of birth control, and the hypertension notifications support document says the feature is not intended to diagnose, treat or aid in the management of hypertension.
Apple claimed no regulatory clearance for anything it announced on September 9, on either watch. That includes the new sensing, the Readiness score, Health Age and the Movement Evaluations in the redesigned Health app.
What Apple has not said
- Whether the heart rate accuracy claim was tested on both watches or on one.
- Any per-configuration price for either model, including titanium, ceramic and cellular.
- Whether the larger Ultra 4 case changes optical sensing in any way.
- The Series 11 and Ultra 3 background reading rates behind the 60 times claim.
- Any regulatory clearance for the features announced on September 9.
Both watches reach stores on Friday, September 18, and watchOS 27 arrives free on Monday, September 14.
