Apple Watch Series 12 and Apple Watch Ultra 4 both run what Apple calls the Health Sensing System, and Apple has attached a published sampling rate to exactly two of the things it measures: heart rate every five seconds all day, and heart rate variability every five minutes. Every other sensor on Apple’s list is named without an interval.
Frequency is the headline claim this year, which is why the gap is worth spelling out. Apple describes the new watches as taking 60 times more frequent background heart rate readings than Series 11, and 24 times more frequent HRV readings, with the same comparison made for Ultra 4 against Ultra 3. What the Health Sensing System actually is
Apple describes it as new optical and electrical heart sensors with larger, power-efficient green LEDs. The spec pages list a third-generation electrical heart sensor and an always-on optical heart sensor.

HRV arrives in two forms. Apple names Recovery HRV, which it ties to daily stress and recovery, and overall HRV. A new daytime vitals view sits alongside overnight vitals, which compare Recovery HRV against your own baseline.
The two measurements with a stated interval
Heart rate every five seconds. HRV every five minutes. Those are the only cadences Apple has published for either watch, and both are all-day background figures rather than workout numbers.
They also feed Readiness, the daily score from 0 to 10 that Apple says is built from recent activity, training load, vitals and sleep. It updates during the day and resolves to one of four recommendations: Recover, Pace Yourself, Ready or Go For It.

The health sensor lists are otherwise identical, so the split is about water hardware rather than health hardware. Apple rates Series 12 to 50 meters and Apple Watch Ultra 4 to 100 meters under ISO 22810:2010, and only Ultra 4 carries the EN13319 rating for recreational scuba to 40 meters.
The sensors Apple never gave a rate for
Blood Oxygen, wrist temperature, water temperature, the altimeter, the accelerometer, the gyroscope, the ambient light sensor and the depth gauge are all listed by name with no measurement interval attached to any of them.
ECG is listed as an app rather than a background reading, and Apple publishes no interval for it either. Hypertension notifications are the one measurement with a published schedule in Apple’s older documentation: the support page says the feature analyses optical heart sensor data over 30-day evaluation periods. That page still lists Apple Watch Series 9 or later and Ultra 2 or later, and it has not been updated to name Series 12 or Ultra 4.
These are wellness features, and Apple says so
Apple states that the Vitals app is for wellness and not for medical use. Its hypertension notifications document says the feature is not intended to diagnose, treat, or aid in the management of hypertension or other conditions such as blood clots, stroke, atrial fibrillation, congestive heart failure, or high cholesterol.
Apple has claimed no regulatory clearance for anything it announced on September 9. A sampling rate describes how often a sensor reads, which is not the same as a claim that the watch can find an illness. Apple has not made that claim, and the frequency figures should not be read as one. Our event recap covers what was and was not announced.
What Apple has not said
Apple has not published a measurement interval for any sensor other than heart rate and HRV, and it has not published per-sensor accuracy figures for the individual readings. It has not said whether older watches gain the higher sampling rates when they move to watchOS 27, and our pre-event Series 12 preview explains what that update brings elsewhere.
Apple has also published no support tech spec document for Series 12 or Ultra 4, which is where footnotes like these normally live.
watchOS 27 arrives free on Monday, September 14, and both watches reach stores on Friday, September 18. Apple’s support documents for new models usually appear around launch, and that is when any remaining sampling detail would show up.
