Apple Watch Series 12 and Apple Watch Ultra 4 measure heart rate variability every five minutes, and Apple reports the result in two named forms: Recovery HRV, which it ties to daily stress and recovery, and overall HRV. The difference is not the sensor. It is what each number is measured against.
Apple says overnight vitals compare Recovery HRV with your personal baseline, which means the figure is judged against your own history rather than a published range. Overall HRV is the longer-running measure Apple Watch has reported for years, and Apple has not published a comparison rule for it.

The two numbers Apple names
Apple’s wording is short. Recovery HRV is described as covering daily stress and recovery. Overall HRV is listed separately, as the second of the two variants the Health Sensing System produces from the same five-minute readings.

Both come from the same hardware. The Series 12 Health Sensing System pairs new optical and electrical heart sensors with larger power-efficient green LEDs, and Apple gives Apple Watch Ultra 4 the same stack and the same claims.
A personal baseline, not a target number
The important consequence of a personal baseline is that the raw figure is not the point. Two people with the same overnight HRV can sit in different places relative to their own history, and Apple’s comparison is to that history.
It also means the measure needs time before it says anything. A baseline drawn from your own nights cannot exist until enough of those nights are recorded, and Apple has not published how many that is.
How often the watch reads you now
The frequency is the headline change this generation. Apple says the new watches take HRV readings 24 times more often than Series 11 and Ultra 3, at intervals as short as five minutes, and background heart rate readings 60 times more often, at every five seconds.
A new daytime vitals view puts the daytime side of that on screen, and the overnight vitals view handles the comparison against your baseline. Apple has not published the exact window Recovery HRV is calculated over.
Where each number feeds the rest
Vitals is one of the four inputs Apple names behind the daily Readiness score, alongside recent activity, training load and sleep. Apple has not published how heavily vitals counts, or which of the two HRV figures the score uses.
On iPhone, the redesigned Health app lists HRV among the metrics behind Health Age, together with VO2 max, resting heart rate and sleep, and that feature requires an Apple Watch. Apple showed all of it on September 9 as part of the Health app rebuild.
Wellness data, not a diagnosis
Apple is explicit that the Vitals app is for wellness rather than medical use, and neither HRV figure is presented as a test result. A number below your baseline is a number below your baseline, and Apple attaches no clinical meaning to it.
What Apple has not said
Apple has not published the algorithm behind Recovery HRV, the window it covers, the units or ranges it reports, how many nights build the personal baseline, or how far a reading must sit from that baseline before anything is flagged.
It has also not said whether Recovery HRV or the daytime vitals view reach Apple Watch Series 11, Ultra 3 or SE 3 under watchOS 27. Apple lists both under the new watches only.
